Before the Diablo 4 Ancestral Legendries will start dropping, you first need to reach the Torment difficulty level.
As you can see from the image above, you need to be on world tier 4 (Torment) difficulty to unlock the Ancestry item drops from Normal to Legendary.
Ancestral items are enhanced versions of standard and sacred equipment, with higher item levels and base stats. You won't drop holy items when playing the campaign, or even when farming on Nightmare (World Tier 3) difficulty. Of course, these are the best of the best diablo 4 items, so you'll need a long grind to see the Ancestral Legendary item drop.
Follow these steps to unlock the Ancestral Items drop in Diablo 4:
Complete the Diablo 4 campaign/story
Get ready to meet and clear the Cathedral of Light's Capstone dungeon.
With World Level 3 (Nightmare) difficulty unlocked, switch to Nightmare and start killing monsters in the Sanctuary.
Level up to 70.
Go to Steppe and complete the Corrupted Temple Capstone Dungeon on World Tier 3.
Go back to the main city, interact with the world-level statue, and replace it with world-level 4 torture.
Congratulations, you will now see the Ancestry items start dropping. Keep in mind that Torment difficulty offers quite a challenge, so make sure you have a great build and a full collection of sacred items at this point.

There are several ways to get more Ancestry items in Diablo 4. You want to see which activities give you the best drops and the most legendary items. World bosses are certainly a great way to get ancestral drops, but they're very difficult in Torment, and if you're leveling up quickly, there may not be enough high-level players around. To that end, we recommend running endgame campaigns that focus on the ones where you can finish farming Heirlooms the fastest in Diablo 4.
